Approach
Clients will be assigned a Connections Team, whose members will coordinate the plans and services for the client. Philosophically, SPBH believes that the more points of contact a client has during the week, the healthier they will function; organizing a daily and weekly schedule of routines and appointments will assist clients in staying structured, keep them oriented toward their goals, and put them in frequent contact with their support network.
Participation in the program will be dynamic and interactive through 3 avenues:
- The client will come to the SPBH office for services.
- Connections Teams will visit and work with clients in their home.
- Connections Teams will supervise/teach clients as they execute therapy goals and implement learned skills in the community.
SPBH offers Connections programs that are school-based for children and adolescents, adult-oriented for clients living and functioning in the community, and for clients with mental health needs that are transitioning from programs with higher levels of care (Intensive Outpatient and Partial Hospitalization Programs, wilderness therapy, and residential treatment centers). SPBH offers step-down services and represents the least restrictive intervention.

Connections For Transitions In Care
Connections for Transitions is a partnership program for children, adolescents, adults, families, and providers that addresses client needs across the continuum of psychotherapy and behavioral health interventions. This program functions to support the shift from one level of care to another; some clients may be prepared to decrease services based on the reduction of acute symptoms, whereas others may need to increase services as their needs become more complex.
